Job Description

Qualifications:

BA/BSW. Bilingual in Spanish preferred. Work experience in Child Welfare &/or Office of Mental Health System preferred. NYS/CT driver’s license preferred. Ability to travel throughout NYC area & use public transportation as necessary.

Position Summary:

Provides comprehensive case management services for assigned clients including but not limited to advocacy, assistance in problem solving, assessment, conflict resolution, entitlements, training on daily living skills, and referrals. Provides intensive case work services to children and families in order to implement the goals and tasks of the Family Assessment and Service Plan (FASP). Responsible for planning with child’s discharge resource including arranging services in the community for the child’s family members & discharge resource. Responsible for court appearances related to assigned cases.

Position Responsibilities:

  • Schedule casework contacts in accordance with ACS requirements.* 
  • Ensures adherence to CMCS policies and corporate compliance procedures  
  • Provides comprehensive case management services for assigned clients including but not limited to advocacy, assistance in problem solving, assessment, conflict resolution, entitlements, training on daily living skills, and referrals.  
  • Provides intensive case work services to children and families in order to implement the goals and tasks of the Family Assessment and Service Plan (FASP). 
  • Work with the Sociotherapist to provide direct, in home crisis intervention as needed.*  
  • Responsible for planning with child’s discharge resource including arranging services in the community for the child’s family members & discharge resource.*  
  • Responsible for court appearances related to assigned cases.* 
  • Responsible to set up, monitor and help youth maintain family connections through siblings visits and family visits with their biological family or discharge resource.* 
  • Participate in bi- weekly team meetings, staff meetings, clinical conferences, training & other functions as required by the TFFC program.* 
  • Responsible for the timely completion of all required forms/paperwork: FASP’s, plan amendments, APPLA checklists, family portion of the psychosocial summary, court reports including Permanency Hearing Reports, case management tracking, correspondence, progress notes, etc. * 
  • Provide 24-hour on call crisis intervention services through after hours support coverage rotation with other TFFC staff.* 
  • Participate in Family Team Conferences, Service Plan Reviews, case consultations, and other functions as appropriate.* 
  • Report any changes in child’s case planning or behavior to the Program Supervisor.*  
  • Ensure that each child is participating in recreational activities & receives recommended medical service including when appropriate family planning, bill of rights and human sexuality information * 
  • Support & assess youth’s progress in the areas of: education, spiritual / religious development & progress toward Independent Living goals.* 
  • Complete 2 face-to-face school visits per academic year and ensure a current report card is on file.*  
  • Collaborate effectively with other service providers within CMCS and in the community.* 
  • Share work schedule with Program Supervisor and Sociotherapist partner.  
  • Ability to travel throughout NYC area & use public transportation as necessary.*
