Master’s Degree in Human Services Required. Bilingual in Spanish preferred. 2+ years working with similar population preferred and knowledge of CNNX and Child Welfare. Experience facilitating trainings or group meetings strongly preferred. Must be able to successfully complete the Therapeutic Crisis Intervention Course.

Position Summary:

The Family Team Conference (FTC Facilitator)/Trainer is responsible to facilitate all family team conferences for the Horizon of Hope Residential Program.  The FTC Facilitator/Trainer responsibilities includes supporting clients and staff to have collaborative discussions to develop service plans for youth in care to support safety, permanency and well being.  In addition, the FTC Facilitator/Trainer is responsible for developing curriculum to facilitate regulatory and best practice trainings for staff within the Horizon of Hope Program.

Position Responsibilities:

  • Manager the development of the Family Team Conferencing model and facilitation for the HOH program
  • Schedule, plan and facilitate all FTC’s and Service Plan Reviews for youth in the program.
  • Develop collaborative relationships with ACS and DSS stakeholders to support FTC and SPR facilitation in alignment with internal and external regulatory requirements.
  • Provide culturally and trauma sensitive approach to conference facilitation that supports open communication and collaborative decision making.
  • Document all conference activities in CNNX and provide written agreements to all participants
  • Develop and lead staff training to support professional development and regulatory requirements
  • Develop curriculum materials related to regulatory and best practice requirements
  • Collaborate with program leadership on training topics and needs to support program and staff performance
